Start with the basics of what happened
An intake request is not a legal filing and it is not a statement to an insurer. It is a short, plain-language description of a situation so it can be reviewed for possible connection to a participating Florida attorney. The most useful requests answer a few simple questions without any legal vocabulary at all.
Requests that are easy to review usually include:
- When it happened. An approximate date is fine if you do not remember exactly.
- Where it happened. The Florida city or county, and the road or intersection if you recall it. Florida geography matters because participating lawyers cover different regions.
- What kind of crash it was. For example a rear-end collision, an intersection collision, a single-vehicle incident, a hit-and-run, or a crash involving a motorcycle, bicycle, or pedestrian.
- Who was involved, in general terms. How many vehicles, whether you were a driver or passenger, and whether a commercial or work vehicle was involved.
- Whether law enforcement responded. If a report exists, it is enough to say so — you do not need to attach it at intake.
- Whether anyone sought medical care. Say whether you or someone else was seen at a hospital, urgent care, or a clinic. Do not send records or medical identifiers.
- Whether insurance is already involved. A note like "a claim was opened" or "an adjuster has called" is useful context. Do not include policy or member numbers.
- Representation status. Whether you already have a lawyer for this matter.
- Contact preference. Phone or email, and the hours that are realistic for you.

What to leave out of any intake form
Intake needs context, not identity documents. Please do not submit Social Security numbers, driver's license numbers, insurance member or policy numbers, medical record numbers, financial account or card numbers, passwords, or photographs of identity documents. If a form anywhere ever asks you for those things to "verify" a request, treat that as a reason to stop.
Also leave out long medical narratives and clinical documents. "I was treated at an emergency room and I am still having back pain" is plenty for intake review. Detailed medical discussion belongs in a private conversation with a lawyer you choose to speak with, not in a web form.
If you are already represented
Say so in the request. When a consumer tells us they already have a lawyer for the matter, we do not route it to another participating lawyer, and we will not contact the current lawyer for them. If you are unhappy with your current representation or unsure about it, that is a conversation to have with your current lawyer directly.
Passengers, pedestrians, cyclists, and motorcycle riders
Intake works the same way whether you were driving, riding as a passenger, on a motorcycle, on a bicycle, or walking. Describe your role plainly — for example "I was a passenger in the back seat" or "I was crossing on foot at a marked crosswalk" — because that detail changes which participating lawyers are a fit for the matter type. It is not a legal characterization and nothing you write in an intake form commits you to any version of events.
What happens after you submit
You will see a confirmation that the request was received. From there, the request goes through non-legal intake review and then compliance and eligibility review. If a connection is made, a participating lawyer who is lawfully permitted to practice law in Florida may contact you, and their location is disclosed at that point. You decide whether to speak with them, whether to continue, and whether to hire anyone. No lawyer is selected or retained merely because you submitted a form, and you can tell us to stop pursuing your request at any time.
